Medical Forms

Illinois requires health exams for students in the public schools.  A physical exam is required upon school entry if attending preschool and for students in kindergarten, sixth grade, and ninth grade.  Physicals must be done within one year of school entry for each grade level.  Students moving into the district from out-of-state must also have an Illinois physical exam within 30 days of enrollment.  An eye exam is required for children entering kindergarten.  A dental exam is required for children in kindergarten, second and sixth grade.

 

Illinois law requires that students are excluded from school attendance on October 15th if the requirements for the physical and eye exam are not met.  Dental exams must be completed by May 15th.
